秋田市では、高齢者の皆さまが安心して冬を迎えられるよう、インフルエンザ予防接種に対する助成制度を実施しています。当院でも対象の方に接種を行っておりますので、ご利用ください。
📅 当院での接種期間
2025年10月1日（水）～ 2026年12月27日（土）
👥 対象者
以下のいずれかに該当する秋田市民の方
- 接種日に65歳以上の方
- 接種日に60～64歳で、心臓・腎臓・呼吸器・免疫機能に重度の障がいがあり、身体障害者手帳1級相当の方
💰 自己負担額（秋田市の助成あり）
- 課税世帯の方：1,000円
- 非課税世帯の方：500円
- 生活保護受給者等の方：無料
📋Request to Complete the Pre-Visit Questionnaire
• To help reduce waiting time at the clinic,we kindly ask that you complete the pre-visit questionnaire (internal link) in advance,except for the “body temperature” field, which will be measured upon arrival. Please print the completed form and bring it with you on the day of your visit.
If printing is difficult, the same form is available at the reception desk and can be filled out after you arrive.
