📋Request to Complete the Pre-Visit Questionnaire
• To help reduce waiting time at the clinic,we kindly ask that you complete the pre-visit questionnaire (internal link) in advance,except for the “body temperature” field, which will be measured upon arrival. Please print the completed form and bring it with you on the day of your visit.
If printing is difficult, the same form is available at the reception desk and can be filled out after you arrive.
💰 Cost and Subsidy Information
- Standard fee: ¥3,500 (tax included) per dose
- Subsidy: Financial assistance is provided by Akita City.
The amount of the subsidy will be deducted, and the remaining balance will be paid at the reception desk.
Eligible for Subsidy
The following individuals are eligible for financial assistance provided by Akita City:
- Pregnant women (must present a Maternal and Child Health Handbook)
- Children aged 6 months to under 13 years
- Adults aged 65 and older
