📋Medical procedures not requiring written consent🗓️

At our clinic, we handle "items requiring written consent" and "items for which explanation and consent confirmation are obtained verbally and via posted notices" separately. The following examinations and medical procedures may not require the presence of a doctor, and for the smooth progress of treatment, explanations and consent confirmations are provided verbally and through in-clinic postings.
General items
Various consultations, visual inspection, physical examination, temperature measurement, height measurement, weight measurement, blood pressure measurement, nutritional guidance, imaging of affected areas with cameras, etc. (mainly on the body surface)
2) Inspection, monitoring, etc.
Venous blood collection (*1*) (hematology, biochemistry, immunology, virology tests, etc.), urine/stool/etc. testing, microbiological testing (sputum/saliva collection, etc.), physiological function testing (electrocardiogram, respiratory function test, ultrasound, fractional exhaled nitric oxide (FeNO) measurement, etc.), general X-ray imaging, percutaneous oxygen saturation (SpO2) measurement, intradermal reaction testing, infection confirmation for needle-stick/cut wounds and skin/mucosal exposure (HBV, HCV, HIV testing, etc.)
3) Treatment
Venous blood collection, suction of sputum/nasal discharge, wound treatment (cleansing), enema
4) Medication, administration
Routine medication administration, injections, peripheral intravenous catheter insertion (securing an IV line), oxygen administration, etc.
• The medical procedures described above are performed by physicians, nurses, technicians, etc. with a certain level of experience and are considered safe from a general medical standpoint. However, they may still be accompanied by organ and tissue damage, mental (psychological) distress, bleeding, numbness, anaphylaxis, or other unforeseen complications. Although extremely rare, these complications and adverse events can occur at a certain rate, regardless of the practitioner's skill. Furthermore, similar to other medical procedures, symptoms may be subjective and difficult to evaluate with current medical knowledge, their duration may be unpredictable, and some may be permanent.
In such cases, the treatment of complications and adverse events will be covered by standard medical insurance. We appreciate your understanding in advance. If you have any questions, including those about adverse events, please feel free to ask your doctor, nurse, pharmacist, technician, or other staff.
(※1) It has been reported that puncturing (stabbing) peripheral nerves on the surface can cause damage, leading to numbness or motor dysfunction, and that these symptoms may become permanent.

Facility Standard Notification

Our clinic meets the facility standards for pediatric food allergy challenge tests.
We meet the facility standards for multi-item simultaneous detection testing of viral and bacterial nucleic acids.

Regarding medical treatment surcharges

The following medical fees/additional charges are billed at our hospital.
Pediatric outpatient clinic fee
This is a medical institution that practices pediatrics. It is calculated per medical institution when providing outpatient services to patients under 5 years of age.
Functional Enhancement Addition
This is an additional charge for evaluating the primary physician function for patients' initial visits at medical institutions that have registered for the primary physician function. This charge is only applied at the time of the initial visit.
Our clinic offers the following primary care physician functions:
Consultation regarding health management
Referral to specialized medical institutions as needed.
・Guide to handling inquiries during nights and holidays
Consultation regarding vaccinations
・Consultations regarding development and referrals to specialized institutions as needed
Pediatric Antimicrobial Stewardship Support Add-on Payment
This is an additional charge for medical care that contributes to improving patients' and their families' understanding of appropriate antibiotic use.
For acute respiratory infections, acute diarrhea, acute otitis media, and acute sinusitis, the following applies: a charge can be made at the initial consultation only when guidance regarding necessary care and explanations of examination results are provided, and when the content of the explanation is also provided in writing.
General prescription additional fee
For medications for which generic alternatives are available, prescriptions will be issued with the generic name instead of the brand name listed in the drug price standard (the product name of the original brand-name drug). (A "[GEN]" notation will be added before the medication name on the prescription.) This allows you to receive generic medications at the dispensing pharmacy, thereby reducing your medication costs.
5. Outpatient Infection Control Improvement Add-on Payment / Collaboration Strengthening Add-on Payment
This is an additional charge to evaluate measures such as securing consultation space when infectious diseases (such as influenza or COVID-19) are suspected, and improving infection control within the hospital by collaborating with core regional hospitals.
⑥ Overtime Support Allowance 3
For inquiries received by phone from patients who are continuously seen at our hospital, we will respond for a few hours after our regular operating hours. On afternoons when the clinic is closed, holidays, etc., we will use an answering machine to provide the contact information for local emergency medical facilities.
⑦ Electronic Medical Information Collaboration System Maintenance Added Charge (June 2026 -)
Our clinic is equipped to provide high-quality medical care by utilizing medical information obtained through online eligibility confirmation, electronic prescriptions, and other means.
Base-up evaluation fee
Our hospital has been billing for a base-up assessment fee since March of Reiwa 8 (2026) with the aim of improving the treatment of medical professionals.
Regarding the statement
We will provide the statement free of charge in accordance with the medical treatment and billing rules.
