🟢 Vaccination Start Date/Eligible Individuals
- From Wednesday, October 1st,
・Those aged 65 and over
We will begin regular vaccinations for individuals aged 60 to 64 with certain underlying conditions (heart, kidney, or respiratory dysfunction, or immunodeficiency due to HIV) and those equivalent to Class 1 disability certificate holders.
💰 Vaccination costs and subsidies(As of September 13, there is no official announcement, but it is likely as follows)
- The cost per dose at our clinic for fiscal year 2025 will be ¥15,300.
- The amount payable at the counter, after deducting the subsidy from the city, is as follows.
- Households subject to tax: ¥5,600
- Households exempt from taxes: ¥4,100
- Welfare recipients, etc.: Free

The regular vaccination period is from Wednesday, October 1, 2025, to Monday, March 31, 2026.

We plan to use Pfizer's Comirnaty®.
📋Request to Complete the Pre-Visit Questionnaire
• To help reduce waiting time at the clinic,we kindly ask that you complete the pre-visit questionnaire (internal link) in advance,except for the “body temperature” field, which will be measured upon arrival. Please print the completed form and bring it with you on the day of your visit.
If printing is difficult, the same form is available at the reception desk and can be filled out after you arrive.
Regarding simultaneous vaccinations
• It is possible to receive the COVID-19 vaccine and the influenza vaccine at the same time.
• If you wish to receive both vaccinations simultaneously, please select "Simultaneous COVID-19 and Influenza Vaccination" when booking.
Regarding the applicant's proof of identity
• For those aged 60 to 64 with underlying health conditions, please bring documentation that confirms your name, the name of your disability, and its grade (e.g., a Physical Disability Certificate) when you receive the vaccination.
