👥 Target audience
Residents of Akita City who fall under any of the following categories
- Those aged 65 or older on the vaccination date
- Those who are 60 to 64 years old on the vaccination date and have a severe disability of the heart, kidneys, respiratory organs, or immune system equivalent to a Grade 1 physical disability certificate.
💰 Out-of-pocket amount (with subsidies from Akita City)
- Taxable households: 1,000 yen
- Tax-exempt households: 500 yen
- Public assistance recipients, etc.: Free
📋Request to Complete the Pre-Visit Questionnaire
• To help reduce waiting time at the clinic,we kindly ask that you complete the pre-visit questionnaire (internal link) in advance,except for the “body temperature” field, which will be measured upon arrival. Please print the completed form and bring it with you on the day of your visit.
If printing is difficult, the same form is available at the reception desk and can be filled out after you arrive.
